This website is operated by Holland Bloorview Kids Rehabilitation Hospital Foundation. Holland Bloorview Foundation is committed to protecting the privacy of those who use our website.

The purpose of this Privacy Statement is to inform you about the types of personal information that we collect, use and disclose in connection with the Change for Kids program.  It explains how we use and disclose that information, the choices you have regarding such use and disclosure, and how you may correct that information.

From time to time, we may make changes to this Privacy Statement.  The Privacy Statement is current as of the “last revised” date which appears at the top of this page.  We will treat personal information in a manner consistent with the Privacy Statement under which it was collected, unless we have your consent to treat it differently.  This Privacy Statement applies to any information we collect or receive about you, from any source.

Personal Information

“Personal Information” is generally any information that is about an identifiable individual, such as your name, address, telephone number, screen name and e-mail address.  Throughout our site, you may be asked for personal information so that you may register for a promotion to win prizes, fill out a survey or for other reasons.

Collection and Use of Personal Information

We always collect your Personal Information by fair and lawful means.

We generally obtain your consent prior to collecting, using or disclosing your Personal Information for any purpose.  Personal Information is collected only if you supply it voluntarily during the process of creating a HBYOUfie, signing up for future information, making a donation, or as part of contacting us with a comment, question or complaint.

We identify the purposes for which we use your Personal Information at the time we collect such information from you and obtain your consent, in any case, prior to such use.  We generally use your Personal Information for the following purposes (the “Purposes”):

  • to register you for, and facilitate your participation in, the Change for Kids program, including submitting a HBYOUfie, registering to qualify for a prize, and publicising the names of prize winners;
  • to administer our activities related to the Change for Kids program;
  • to respond to your comments, questions or complaints;
  • such other purposes as may be permitted or required by applicable law;

And, only if consent has been provided with a checkmark in the applicable box when registering on the Change for Kids website:

  • to provide information on our future fundraising and other foundation activities;
  • such purposes for which we may obtain consent from time to time.

Disclosure of Personal Information

Holland Bloorview Foundation does not share any Personal Information with any third parties, except to third party service providers with whom we have a contractual agreement that includes appropriate privacy standards, where third parties are assisting us with the Purposes – such as service providers that provide data storage or processing.  We do not rent, sell, or trade Personal Information.

Retention of Personal Information

We retain Personal Information only as long as necessary for the Purposes for which it was collected, or as required or permitted by law.  We will keep the Personal Information that we collect either at our offices in Toronto, or at the offices of a service provider in the GTA.

In addition, Personal Information may be processed and stored for the Purposes in the United States, and if so, will be subject to the laws of the United States, and may be subject to disclosure to the governments, courts or law enforcement or regulatory agencies pursuant to the laws of the United States.

We provide links to third party websites as a convenience to users of our site. We do not operate these sites and these links are not intended as an endorsement of the linked websites. The linked websites have separate and independent privacy statements, notices and terms of use, which should be read carefully as part of accessing these sites. Holland Bloorview Foundation does not have any control over such websites, and therefore has no responsibility or liability for the manner in which the organizations that operate such linked websites may collect, use or disclose, secure and otherwise treat Personal Information.

Security

Holland Bloorview Foundation has implemented measures designed to protect against the theft, loss, unauthorized access, use, disclosure or alteration of Personal Information in our custody and control with security measures appropriate to the sensitivity of the information, including through the use of reasonable physical, organizational, and technological safeguards and appropriate training of employees.

Holland Bloorview Foundation’s third party service providers are obligated to maintain confidentiality and security of the Personal Information transferred to them and to perserve the intent of this Privacy Statement.

Opt-Out

If you no longer wish to receive information from us about our activities or events, you may be removed from our mailing database by following the appropriate link included in such emails. You may also request removal from future emails or direct mailings by telephoning 1-416-424-3809 or by sending an email to our Privacy Officer.

Accuracy

We will ensure that your Personal Information is kept as accurate, complete and up-to-date as possible.  We will not routinely update your Personal Information, unless such a process is necessary.  We expect you, from time to time, to supply us with written updates to your Personal Information, when required.

Access, Corrections, Complaints

If you want to access any Personal Information that we have collected, used or disclosed, we will provide you with any such Personal Information subject to applicable law.  We will make such Personal Information available to you in a form that is generally understandable, and will explain any abbreviations or codes.

At any time, you can challenge the accuracy or completeness of your Personal Information in our records.  If you successfully demonstrate that your Personal Information in our records in inaccurate or incomplete, we will amend the Personal Information as required.  Where appropriate, we will transmit the amended information to third parties having access to your Personal Information.

You may also submit a written privacy complaint to the Privacy Officer.  Within a reasonable time after receiving the written complaint, the Privacy Officer will conduct an investigation into the matter, and will respond to your complaint and take any appropriate measure necessary to rectify the source of the complaint.

We will attempt to respond to each of your written requests not later than thirty (30) days after receipt of such requests.  We will advise you in writing if we cannot meet your requests within this time limit.  You have the right to make a complaint to the federal Privacy Commissioner or the Ontario Privacy Commissioner in respect of this time limit.

In responding to your requests, we may require you to provide sufficient identification to permit access to the existence, use or disclosure of your Personal Information.  Any such identifying information shall be used only for this purpose.

Children

Holland Bloorview Foundation is committed to protecting the online privacy of children.  Children 17 years old or under will require parental permission to enter the website.  No Personal Information will knowingly be collected by us from a child under the age of 18.
